I have a serious question for Mormons (and everyone else) regarding perfectionism?

We are already perfect. God is perfect and so are we. We perform perfectly what we need to learn and to grow towards being even better. You need to look back on the things you feel are failures or when you weren't perfect and see what it was that you needed to learn from that situation. The situation happened perfectly and you performed perfectly to learn a lesson. Self esteem comes from knowing that you learned from the situation, you are still perfect as God created you (actions and cirstances do not make the person), and that the situation was perfect to teach you what needed to be learned.
